Establishing Care
Whether you are expecting a new baby, have recently moved to our area, or are looking for a new doctor for yourself or your children, we thank you for trusting us with your health. Atlantic Internal Medicine and Pediatrics welcomes new patients of all ages.
We’re excited to have you join our practice! To ensure a smooth transition of your healthcare, please review the following information:

Call your insurance company to verify that Dr. Adam Kurish, Madeline Grandin or Sarah Boyatsis is listed at our practice address, and to change your Primary Care Provider (PCP) to them. If your insurance plan is not on our list, we are always happy to see you out-of-network or on a cash-pay basis.

Please contact the office during business hours and our staff will happily assist you with registration to become a new patient. We require you to contact our office prior to release of medical records.

Submit the completed form to your previous Doctor's office. Our goal is to provide seamless thorough care, for this reason our providers require that medical records be reviewed prior to your first appointment so that they can start working on your health prior to you walking through the door.
